petition for reinstatement after academic disqualification
I am attaching the requirements of the University. I am asking for help with a reinstatement letter so that I can get back into school this summer session and the fall of 2019. I failed two courses and passed two courses, in the Fall of 2018 and I failed four courses in the Spring 2019. I made four F’s and 1 A. I am asking for reinstatement based on the fact that I have matured and know that I have to do what is necessary to get the courses completed and get my degree. I am currently a senior, majoring in Liberal Studies. I have only 16 credit hours left to complete the degree. I did not do well academically because my mother was sick and diagnosed with a heart disorder, which she had to have a procedure to remove a blockage. I loss my father to lung cancer and my grandfather who was very close to me, he too succumbed to throat (oral) cancer. The losses were surmountable and clouded my judgement and ability to perform academically and athletically, as I was on a football scholarship as well. The demands of academics and athletics were a lot to task out for me.
Classes proposed to be taken in the Summer 2019 are:

In order to ensure my success I will utilize the following resources on campus: Tutoring Support Services, Library Services, Writing Center, Dean of Students, Center for Disability Access and Resources( Counseling, if needed to assist with coping and adjustment), Career Services, Office of Multicultural Affairs, Student Support Services Counseling and Testing Center and Financial Aid.

The actions that led to my probation were due to the fact that I have experienced problems with being able to concentrate, because of home sickness, processing family losses (deaths), not being organized and not reaching out to the designated resources to assist with my adjustment and settle me to be able to perform academically. The family loss I experienced were significant persons who impacted my life tremendously. I have been working to learn better adaptation skills and adjustment skills in order to deal with the losses and move on with my life. My dad is expired and my grandfather shortly after that, they both were an integral part of me, my granddad lived with us and he actually died in my arms waiting on the ambulance. I have been working with a counselor around these issues of loss.
How do you intend to change these habits and behaviors in the coming term?*
I am near the end of my degree and I need the degree in order to sustain myself and hopefully one day my family. I have matured a lot with what I have encountered over the past couple of years. I know that I have to study, I have to reach out to my professors and tutors and other means of help when I am struggling with course work. I am motivated and charged with no option for failure. I cannot disappoint myself, because this is my life and I cannot treat it like a hopper of vague chances. I have the tools I need to succeed as well as the capacity to do the work as assigned.
